Formation Counseling Services' mission is to partner with churches and other ministries to provide counseling services that inspire healing, growth, and renewal.
Services Offered
Because we have such a large clinical staff, we are able to competently treat a wide variety of presenting problems and issues, including: * Anxiety * Depression * Grief and loss * Anger management * Stress management * Spiritual issues * Pre-marital counseling * Marital and relationship difficulties * Sexual difficulties in marriage * Divorce recovery * Conflict resolution * Life transitions * Parenting issues * Child and adolescent behavior problems * Attention deficit disorders * Bipolar disorder * Trauma and disaster recovery * Family of origin issues * Chemical addictions (in conjunction with other treatments) * Non-chemical addictions, including sexual addiction * Eating disorders * Abusive relationships/Domestic violence * Codependency * Career issues and job stress * Psychological aspects of medical issues (such as chronic pain) * Men's issues * Women's issues
Some of the services our highly educated, trained and experienced psychologists, social workers, and counselors offer are: * Individual, couple, and family counseling for adults, adolescents, and children * Marital and relationship counseling * Pre-marital or pre-engagement testing and counseling * Specialized counseling from a Christian perspective * Consultation with pastors regarding mental health issues * EMDR
Fees and Payment
We make every effort to provide our services to persons regardless of the level of their financial resources. This is a central part of the mission of our agency. Toward this end, we do three things: 1. We accept many insurance plans which help make our services affordable. 2. We waive a portion of our standard fee, in situations of financial hardship, for those who are self-paying for their services (not using insurance). 3. We also offer two scholarship funds which are provided by the generosity of donors to our agency. These two scholarship funds, the Through the Roof Fund and the At the Well Fund, help people afford our services when the financial assistance provided by No. 1 or No. 2 above does not meet their need.
We are pleased to service the following insurance plans as contracted, in‑network providers: Aetna, Beacon Health, Blue Cross/Blue Shield, Cigna, Medicaid, Molina, OptumHealth, Presbyterian Health Plan, TRICARE, and United Health Care (United Behavioral Health).
Please note that not all of our providers are covered by every plan above. Please also note that some health insurance plans do not include coverage for mental health benefits and some do not include coverage for certain services (such as marital counseling).
We consider it a privilege to assist you in determining what insurance coverage you would have for our services and for which therapists. You are welcome to call us at (505) 821-3628 (Monday through Friday) with your insurance information and we will research your coverage for you. We are also able to service a wide variety of insurance plans as out-of-network providers.

Services Offered To: Adults Couples Teens Children
A community counseling agency is a group therapy practice that offers affordable mental health services. Most are independent non-profit agencies, state agencies, or publicly-funded agencies. A few may restrict who is eligible for services, so it is a good idea to check when you call.
Community counseling agencies are generally more affordable than other therapy providers, but how much more affordable they are will depend on whether you qualify for a discount or sliding scale fee (or if they accept your insurance). Many, but not all, have a policy that they won't turn away anyone due to inability to pay. You should ask about their fees when you first call.
Expect to talk to kind people who want to help you find the care you need. Most community agencies strive to connect you with a live person within 24 hours, if not immediately. If you're asked to hold or leave a message, don't give up; just leave a message and wait. You should hear back pretty quickly.
Most agencies try to set up an initial assessment appointment within a week (some do within 24 hours), though the waitlist to start therapy is usually longer—about a few weeks on average. If you're not eligible or if the agency is not right for you, it's usually still worth it to call or drop in, because staff are knowledgeable about local options and can often refer you to one.
