Services Offered

Behavioral Health, sometimes used interchangeably with 'mental health', covers the emotional and mental well-being of a person. It encompasses issues such as depression, anxiety to relationship issues.

Fees and Payment

Payment Options
Patients that have an outstanding balance on their account have three different payment options to choose from: (1) pay their account in full and receive a discount, (2) make automatic payment arrangements with a credit card or bank account information, (3) make arrangements to call/mail in monthly payments. Appropriate payment arrangements must be made on outstanding balances.
1: Same-Day Discount - Pay account in full and receive a 10% discount or pay account in full on specified date with no discount.
2: Finance - Waive finance charges by scheduling automatic monthly payments deductions that can be charged to your credit card, debit card, checking account or savings account; on any day of the month. A minimum of 10% of the outstanding balance must be paid monthly with this arrangement.
3: Payment Plan - Arrange a call/mail in payment plan. Payments must be at least 10% of the outstanding balance and be made by the final day of each month. An additional one-time 18 percent finance charge is placed on the account with this option.

Sliding Fee Scale Program

The center will take a holistic approach to maintaining a healthy community through education, prevention, and a community networking system.

Thank you for choosing the Bear Lake Community Health Centers to serve you and your family's healthcare needs. Our services include medical, dental, and behavioral health. We are federally qualified health centers which enable us to offer all of our qualifying patients a sliding fee scale. The sliding fee scale is a financial program for families and individuals based upon household size and annual income. Additionally, the pharmacy offers special pricing for the patients of Bear Lake Community Health Centers.

VIP Discount Program
Patients eligible for the VIP Discount Plan are patients who have insurance and self pay patients. Bear Lake Community Health Centers Discount Health Care Plan is not insurance. The plan is a health discount program for medical, dental, and behavioral health services. The VIP Discount Health Care Plan provides a discount of 30% on patient responsibility charges once the co-pay obligation has been met.

The VIP Plan applies to the following:
- Medical procedures
- Medical office visits
- Dental office visits and procedures
- Behavioral health visits

The VIP Plan DOES NOT apply to the following services and charges:
- Patient co-pay obligations
- Medical labs
- Dental labs
- Durable medical equipment
- Pharmacy
- Outside services

The VIP Plan is not available to be used with other discount programs and does not apply to patients on our Slide program.
