Behavioral Health - Our dedicated team of professionals is here to provide mental health counseling services in a confidential setting. We offer individual, family, and group counseling appointments. Our behavioral health team works closely with our medical and dental team to make sure each patient receives both the mental health support and physical care needed for complete well-being. Counseling is offered to patients of all ages.

Depression and Anxiety Screeners - There are tools that we use at the clinic that help us to screen for the presence of anxiety, depression, and other mental health symptoms. These tools are only used to determine whether you may be experiencing some symptoms, but they are not used for diagnosing mental health disorders. We have provided a couple links for some screening tools for your convenience.
Substance Abuse - Is alcohol, drug, or other chemical dependency impacting your life, or the life of a loved one? We offer outpatient behavioral health and substance abuse counseling to meet your needs. Call to learn more and make an appointment.

Fees and Payment
We accept most types of insurance including Medical Assistance, MinnesotaCare, BadgerCare, and Medicare. Please call us if to find out if we accept your insurance. All insurance co-payments are required to be paid at the time of the visit. We accept cash, check or credit card.
All patients without insurance are requested to meet with the Health Care Access Office staff to determine eligibility for public health insurance or other programs that will help pay for your health care services.
If you do not have medical or dental insurance or if your insurance does not cover certain services, you may be eligible to enroll in our Sliding Fee Program. Fees are discounted based on household size and income. You will be asked to provide proof of household income, usually the 1040 tax form from the most recent year, a copy of your W-2s, Social Security or pension statements, pay stubs, and any other income received by your household prior to obtaining services. If you do not qualify for the Sliding Fee Program, you will be billed for the full amount.
All patients on the Sliding Fee Program are required to pay a minimum fee at the time of each visit.

A community counseling agency is a group therapy practice that offers affordable mental health services. Most are independent non-profit agencies, state agencies, or publicly-funded agencies. A few may restrict who is eligible for services, so it is a good idea to check when you call.
Community counseling agencies are generally more affordable than other therapy providers, but how much more affordable they are will depend on whether you qualify for a discount or sliding scale fee (or if they accept your insurance). Many, but not all, have a policy that they won't turn away anyone due to inability to pay. You should ask about their fees when you first call.
Expect to talk to kind people who want to help you find the care you need. Most community agencies strive to connect you with a live person within 24 hours, if not immediately. If you're asked to hold or leave a message, don't give up; just leave a message and wait. You should hear back pretty quickly.
Most agencies try to set up an initial assessment appointment within a week (some do within 24 hours), though the waitlist to start therapy is usually longer—about a few weeks on average. If you're not eligible or if the agency is not right for you, it's usually still worth it to call or drop in, because staff are knowledgeable about local options and can often refer you to one.
