Services Offered

Once your primary care provider refers you to the behavioral health program, you will be eligible to see a Behavioral Health Specialist who will maximize your overall wellness by:
- Integrating your behavioral health needs with your medical care
- Completing screenings, referrals, and assessments
- Providing individual and/or group counseling

You will also have access to a Patient Navigator who will:
- Assist in eliminating barriers to care (like transportation needs)
- Identify resources for additional community support
- Maintain weekly contact with you for ongoing follow up care

Fees and Payment

You are eligible to receive care through our Behavioral Health program if you are seeing a Community First Health Centers provider - just ask your doctor for a referral.

Most insurance plans are accepted, but your individual costs will be determined by your insurance company. We also take pride in offering this program to clients without insurance through our Sliding Fee program, so you can get the care you need at a price you can afford. You can direct all questions regarding eligibility to the Patient Navigator.
