Iowa Mental Health Services Guide

Many people don't realize that publicly-funded mental health services are available in their state. People looking for free or low-cost counseling often think their only options are counselors in private practice and don't know that publicly-funded providers in their communities may also offer counseling services. 

 

While state-based programs are not for everyone, they're often a great place to start for people who face geographic or financial barriers to therapy. Intake specialists at community mental health programs can help people learn whether they qualify for state-funded services and can refer people who don't qualify to other low-cost programs that may be able to meet their needs.


 

When Should You Go to a State Mental Health Program?

 

Few private mental health providers are able to immediately serve people in crisis, while most state mental health programs, including Iowa's, have crisis response systems that help people get mental health care quickly in an emergency. This usually makes public mental health services the best option if you're having a mental health crisis and need help right away. The people who answer crisis lines can provide caring attention and support as they help you determine the best response to a crisis, whether it's inpatient treatment or an appointment with a counselor.

 

Publicly-funded outpatient mental health programs in Iowa are called community mental health centers (CMHCs). In addition to affordable therapy, CMHCs provide specialized and intensive services that aren't available anywhere else or that are hard to find in private clinics, like case management, psychosocial rehabilitation, and day treatment programs. Consider going to an CMHC if you need intensive treatment, live in an area with limited mental health resources, or can't access mental health care in the private sector due to your diagnosis or financial situation.


Who Is Eligible for Public Mental Health Services in Iowa?

 

In the last decade, Iowa's public mental health system has undergone massive change, and the system continues to evolve. Originally, each individual county in Iowa was responsible for its own public mental health system, and the rules, procedures, and eligibility requirements for public mental health services in Iowa varied from county to county. The disparities this caused across the state inspired Iowa officials to shift to a regional model starting in 2014.

 

This transition is ongoing and eligibility requirements still vary across programs. In general, Iowa's public mental health system focuses on serving people with incomes that place them at or near the Federal Poverty Level (FPL). For example, the Southwest Iowa Mental Health and Disability Services (MHDS) Region indicates eligible individuals must have a gross income at or below 150 percent FPL to qualify. The Southern Hills MHDS Region lists the same requirement (PDF).

 

These MHDS regions, and others that list specific eligibility requirements, also list clinical criteria that people must meet to be eligible for publicly-funded mental health care. However, these are not as restrictive as clinical eligibility requirements in many other states. The clinical criteria require that a person have a condition listed in the Diagnostic and Statistical Manual of Mental Disorders (DSM), but not necessarily one that qualifies as a Serious Mental Illness (SMI).

 

You may still be able to get mental health services at a publicly-funded program in Iowa even if you do not meet the above criteria. None of the publicly-funded providers in Iowa are directly run by the state or by MHDS regions; instead, they are independent programs that are contracted by the state to provide services that meet state criteria. These providers include CMHCs, clinics, and non-profit organizations, and each has their own eligibility criteria. If you don't qualify for state financial assistance, you may be able to get affordable mental health services at one of these providers by using your insurance or paying a sliding-scale fee.


How Can You Find Out More About Local Programs in Iowa?

 

The best way to find out about your local CMHC or another publicly-funded program in Iowa is to call the program directly. Through phone or in-person evaluations, intake counselors can help you find out if you qualify and can give you information about how to get the care you need. You can also use them for information and referrals even if you're not in crisis. The webpage for the crisis line includes a link to a live chat feature. Federally qualified health centers (FQHCs) are another option for public mental health care in Iowa. These federally-funded programs provide medical and mental health services to people in underserved communities. Their goal is to deliver high-quality coordinated care to people with complex needs and to link behavioral healthcare with primary medical care. Each FQHC accepts Medicaid and Medicare and offers sliding-scale fees to people without insurance. How Does Iowa's Public Mental Health System Work?

In the 1960s, Americans started thinking differently about how to treat mental health conditions. New laws required state and local governments to establish community mental health programs as alternatives to institutionalization for people with serious mental illness. Many states' public mental health programs trace their origins back to this period. Iowa is no exception. Many of its CMHCs were founded in the period from the early 1960s to the early 1970s using funds from the Community Mental Health Act, which was signed into law by President John F. Kennedy in 1963.

 

While Iowa has a long history of providing community-based mental health services through CMHCs, it lagged behind other states in building a unified public mental health system. For decades, Iowa left it to each county to fund and define the rules for public mental health services in that county. This led to significant disparities in funding and access to care across the state. Iowa did not start building a statewide mental health crisis system until 2008 and did not develop a unified regional mental health system until 2014

 

In response to ongoing issues and dissatisfaction with its system, which has the lowest number of state mental health beds per capita in America, Iowans continue to work on improvements. Serious adult mental health reform laws were signed into law by Governor Kim Reynolds in 2018 and were followed by reforms for the children's mental health system the next year. These laws are still not fully funded and advocates continue to press for funding and additional reforms to improve access to mental health care in Iowa.

 

Currently, public outpatient mental health services in Iowa are managed on the state level by the Division of Mental Health and Disability Services (DMHS), which directs 14 DMHS regions. Each MHDS division oversees the provision of mental health services in the counties that compose that region. No mental health services are directly provided by MHDS. Instead, services are provided by a network of independent CMHCs, which are supported by additional clinics and programs that have been approved to deliver publicly-funded mental health care.

 

According to the Substance Abuse and Mental Health Services Administration (SAMHSA), only 48.5 percent of people in Iowa who have mental health conditions get treatment for them. One reason for the care gap is that people aren't aware of their options for affordable mental health care, including CMHCs and other publicly-funded programs.
